F.O.R.M. BASKETBALL ACADEMY TRAVEL TEAMS

F.O.R.M. Basketball Academy is a program designed to help players achieve their goals both on and off the court. Our program is player driven and we empower our student-athletes to work hard, develop a strong work ethic and buy in to a team-first culture, while showcasing their skills and abilities. We ensure these are done through weekly practices, film sessions, team meetings, player mentoring and continuous on-court and e-learning. This contract keeps both player and coaches accountable to one another and to the team.

Toronto - Sunday September 18 & 25, 2022

6:00-8:00 pm

University of Toronto Athletics Centre - 55 Harbord St., Toronto, ON

Kitchener - Sunday September 18, 2022

6:00-8:00 pm

Impact Training Centre - 10 Executive Pl., Kitchener, ON

Team Fees:

Shall be determined each season of play and be given out during the tryout process, ONLY.

Practices:

1-2 practices will be held every week for the duration of the fall season. Practice dates/times can be found on our Instagram page or email coaches@formbasketball.com

Rep Team Player Package (all Under Armour F.O.R.M. gear):

Each player receives 2 sets of uniforms, track suit, shoes, backpack, long sleeve warm-up, short sleeve t-shirt, reversible practice jersey & shorts, slides, entrance to tournaments, Basketball BC insurance and much more!
