Q: Is there an age limit for my son or daughter?
A: F.O.R.M. is geared towards boys and girls in Grades 7-12 who have a love for basketball and to develop their skills in a fun and safe environment; with fundamentals at the root of all sessions. USports, NCAA, CCAA & Post-Secondary players are welcome to train with the F.O.R.M. during their respective off-seasons. Email or DM us on Instagram
Q: Where are locations for training sessions or practices held?

Q: Can I register my child for multiple terms or multiple packages?
A: Yes. Players can register for multiple terms and/or multiples packages - Eg #1: Elite 1 on 1 5-Pack + Group Training 5-Pack = $1,000.00 + GST Eg #2: Elite 1 on 1 (3-Month) + Group Training (3-Month) = $1,730.00 + GST
Q: Is there a refund policy?
A: Refunds are ONLY available on registered 1 Year terms. Refund policy is as follows: 6 months of training MUST have been completed. The remaining 6 months is charged at a rate of $50.00/month for each remaining month, plus $100.00 Admin. Fee
Exception: a player who is registered for a full year and needs to take time away (Eg: academics reasons, injury and/or extended illness), their membership can be put on hold once ONLY for a MAXIMUM of 8 weeks; time away will be added to the end of that year (Eg: player starts annual term in September, is away for 2 months, their term now expires in November of that following year). A player who is registered for term sessions (Eg: 3 months) will not receive a refund, but may put their membership on hold once ONLY for reasonable and/or extenuating circumstances - MAXIMUM of 8 weeks in length (refer to examples above). No refunds are offered for 5-Packs, 10 -Packs & 3-Month Term
For 3x3 League: If any refunds are processed, a $75.00 Admin fee will be applied; voluntary withdrawal does not constitute as grounds for a refund
Please Note:
1. NO REFUNDS shall be given for paid team fees or if a player is dismissed from F.O.R.M. due to bad/inappropriate behavior, physical or verbal abuse, violence or conduct contrary to the core values/mission of F.O.R.M.
Please Note: the above $100.00 Admin. Fee applies in such an event that a refund is warranted from a team fee paid in full (fees are not in arrears) and is offered by Administration.
As a general rule of thumb, refunds are not offered for team fees, if a player quits the team, misses games, and/or practices, suffers injury/injuries, is in arrears or is dismissed from the program
2. NO RERUNDS shall be given if a player injures themselves while playing for another team (Eg: another club team)
Q: What if my son or daughter can no longer attend F.O.R.M., can I still get a refund?
A: Please see above.
However, case by case situations may be considered at the discretion of F.O.R.M. For your case to be considered, a letter MUST be submitted in writing to payments@formbasketball.com ATTN: Head of Finance & Administrator
Q: Are training sessions/the Training Academy different than the F.O.R.M. Travel/Club team(s)?
A: Yes. Training sessions are solely for skills development. F.O.R.M. offers Elite 1 on 1 and Group training. Travel/Cubs teams partake in games and tournaments. Players can simply choose to train via our Training Academy and NOT play on the Travel Team or can do both. Therefore, both are billed separately and have different fees but can appear on the same invoice (if requested).
Q: Why is there a player cancellation deposit?
A: This helps protect the integrity of the program and holds players accountable; ensuring that they show up to all training sessions. If a player is unable to attend a training session/practice, 24 hours notice is to be given to either their Coach(es) and/or F.O.R.M. Administrator via email ONLY, with the reason for the absence.
Exceptions: if a player is sick the day of a training session or injures his/herself prior to training, then 6 hours notice MUST be given. Failure to do so, will result in a $35.00 per session fee being levied (taken out of the $75.00 cancellation deposit).
If a player misses a 3rd training session and/or practice without proper notification, they are in jeopardy of being dismissed from the Academy.
Q: If my son or daughter is late to a training session or team practice, will they be able to participate?
A: This is at the discretion of the Coach(es). Players are to ensure that they are at any training session or practice at least 15 minutes prior to the start. A Coach may refuse a player joining in the training session or practice, as it may be viewed as disruptive to the rest of the group or team.
Q: What happens when my son or daughter's 3-Month Term/1-Year Term or Pack is about to expire?
A: With 60 days left in a term, the Administrator will send out a reminder email of the pending expiration. At this time, parents have the opportunity to renew early with the possibility of a renewal discount being offered. With 30 days left in a term, another renewal email along with invoice will be sent out. At this time, that renewal payment will be due and MUST be paid in full (unless other arrangements have been made). For 5 & 10-Packs, renewal requests/invoices will be sent out after 50-60% of sessions have been completed; payments are to be made upon receipt or by the specified due date. Please note: if you wish to NOT renewal the existing term and want to change to a different term, you MUST do so in writing upon receipt of renewal requests/invoices within 2-3 business days to payments@formbasketball.com. Late payment charges will be administered for any overdue invoices and/or payments not made by deadlines - see below for late payment fees.
Q: Are there late fees?
A: Yes. If a payment is not made by the specified due date, a 2% per day charge (of amount owing) will be levied up to and including the 7th day. At this time, if payment has not been received, the full/total amount is due - any payment arrangements are VOIDED. A player will be unable to register for the following term and/or may not be able to continue training/practices in that current term or season until accounts are brought up to date. Q: What happens if a session is cancelled?
A: We will do our best to adhere to the given schedule. However, circumstances arise that are out of our control and/or inclement weather occurs (Eg: track or outdoor sessions). If such a need arises that a session needs to be cancelled, we will do our best to notify players and parents via email. Q: How long are training sessions and practices?
A: training sessions = 1 hour - 3 hours (depending on the day/session)
Practices = 1.5 hours - 2 hours
Q: Does my son or daughter need to attend all training sessions and practices?
A: It is beneficial to players that they attend as much training sessions as possible in order to see optimal improvement and results. ALL team practices are mandatory
Q: My son or daughter did not get playing time in a game. Can I talk to someone about this?
A: As an Academy policy, playing time IS NOT a topic of discussion with the Coaches, Executive and/or the President. Playing time is at the sole discretion of the Coach(es). We encourage and empower players ONLY to discuss this with their respective Coach(es). Attendance at practice, attitude, respect, behavior, effort level and knowledge of plays/schemes will be the determining factors for playing time.
Q: Is F.O.R.M. affiliated directly with any post-secondary institutions?
A: No. We are not directly partnered with post-secondary institutions. We do our best to keep keep neutral, while encouraging our athletes to seek post-secondary programs and situations that work best for them. We will provide resources and the necessary information in order for our athletes to make informed decisions. We will do our best to speak with coaches at the next level and provide feedback to our athletes.
Q: Does F.O.R.M. provide scholarships, bursaries, grants or Financial Aid?

Q: With relation to COVID-19 and team or training gear, am I able to return any portions at the end of the season?
A: No. Regardless of if the UA gear has been worn or not, we CANNOT accept any gear back that has been given to players
